Comments:
An unannounced monitoring inspection was conducted on 11/16/2023. The inspector was on site from approximately 3:30 pm-5:30 pm. There were 77 children present, ranging in ages from 4 to 11 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 7 child records and 7 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-530-A-1
Description: Based on review of records and interview, the center did not ensure that at least one staff in each classroom or area where children are present had current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration.

Evidence:
1. The record of staff #4 contained a current CPR certification. The certification was obtained through a company that offers online only certifications. Certifications are required to include an in-person competency demonstration.
2. Administration stated that staff #4 supervised children independently at times and acknowledged that the certification did not meet the requirements.

Plan of Correction: Director will ensure there is a cpr certified person/employee in each group. Director is scheduling staff #4 for our upcoming cpr class to get online and in person training done. Director will ensure to track each employee's training and certifications to ensure compliance.
