Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on June 5, 2020 and concluded on June 5, 2020. The director was contacted by telephone to initiate the inspection. There were 13 children present and 15 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. If there are any questions, please contact karen.dickens@dss.virginia.gov.
Based on record review and interview, one of two employees did not complete a sworn statement or affirmation prior to or on the first day of employment. Evidence: 1. Staff #1's date of hire was 02/18/2020. A sworn statement or affirmation document was signed and completed on June 1, 2020. 2. Executive Director stated that a sworn statement or affirmation was completed by Staff #1 on June 1, 2020.

Documentation in files will be reviewed more closely to ensure compliance.
