Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, interview, and a virtual tour of the program.
A monitoring inspection was conducted on December 29, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were two children present, age two years old for a total of four points, with one staff supervising. Children were observed sleeping during naptime. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies, preventing the spread of disease and nutrition. Two children's records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on interview with the provider and review of documentation, it was determined that the provider did not obtain documentation of subsequent tuberculosis (TB) test or screenings for caregivers and household members every two years as required. Evidence: Staff #1's most recent TB test was dated 1/20/2018. Household Member #1's most recent TB test was dated 2/6/2018.
Plan of Correction:
New TB tests/screenings will be done in January 2022.
