Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was conducted on February 8, 2021. The director was contacted by telephone to initiate the inspection. There were no children present and 1 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 6 staff and board officer records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of Board Officers records, the applicant did not have a central registry check updated every 5 years.
Evidence: 1. Agent #5 had a search of the central registry dated 1/15/2015.
Plan of Correction:
Agent #5 will have a central registry check done as soon as possible. Director will create a calendar system to alert any due checks needed to monitor for future.
