Comments:
An unannounced monitoring inspection was conducted on 11/22/22 from 9:05am to 11:25am with the Director and Owner. There were 86 children in care, ranging in age from four-months to 12-years-old, supervised by 12 staff. The children were observed playing in structured centers, painting, coloring, playing in ?Homeliving?, and playing on the playground. Six child records and six staff records were reviewed. The Center has 19 staff with current certification in CPR and First Aid, and 19 staff trained in Daily Health Observation Training. Seven medications and corresponding authorization forms were reviewed and the center has three staff with current certification in Medication Administration Training (MAT). The attendance, menu, and emergency drill log were reviewed. The carbon monoxide detector was observed. The most recent Fire Inspection on file was dated 9/27/22 and the most recent Health Inspection on file was dated 5/17/22. Standard #: 8VAC20-780-510-E
Description: Based on review of seven medications and medication authorizations, the center did not ensure that procedures for administering long-term prescription medication were followed. Evidence: Long-term prescription medication use is allowed with written authorization from the child's physician and parent. The authorization for an emergency inhaler for Child A contained documentation that the authorization expired on 11/11/22.

Plan of Correction: We were aware of the expiration date and spoke with the parent who already had an appointment scheduled for 11/21/22. The new authorization form was received on 11/23/22. In the future we will have the parent complete a temporary authorization until the new long-term form is received.
