Comments:
An unannounced inspection was conducted with the Director and Owner, in conjunction with local agencies, on 7/11/2023 from 11:30am-12:30pm and 1:40pm to 3:40pm. The inspection was initiated based on the center self-reporting an injury to a child that required outside medical care while under the supervision of the center. Interviews were conducted; a classroom was observed; one staff record and two child records were reviewed; an accident/incident report was reviewed; and a video recording was reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 7/26/2023. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Description: Based on review of a video recording and interview, the center did not ensure that an infant was protected from an older child. Evidence: On 7/5/23 at approximately 7:45am, Child B, a 22-month-old, accidentally sat on Child A, an eight-month-old, who was sitting on the floor in the infant classroom. The older child appeared to become upset and hit the younger child on the left side of the face approximately 10 times in quick succession with a hard plastic, star-shaped toy, leaving bruises and scrapes on Child A?s face.

Plan of Correction: The 18 month and older children are no longer combined with younger children in the early morning. Staff scheduling has been adjusted for this change. This step has allowed child A and child B to be with their specific age groups.

An updated request has been sent to the parents for drop off times for their child on the younger side. This will help to ensure any surprise drop off times to keep children separated.
