Clarified the requirements for Tuberculosis screening / test for those who start working after 10/13/2021.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person of the program. A renewal inspection was initiated on September 22, 2022 and concluded on September 22, 2022. There were 21 children present, ranging in ages from 3 to 5, with 3 staff and 3 parent volunteers supervising.. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff /parent volunteer records were reviewed. Inspection was amended on 9-22-2022. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of 9 volunteer and staff records and staff interviews, the licensee did not ensure that each staff member had obtained a background check in accordance with the requirement and every five years thereafter.
Evidence: Staff #8 does not have an updated Central Registry (CPS) finding as the most recent is dated 3-4-2017. The CPS check shall be updated every 5 years from the date of the last check.
1. Director contacted board member informing them of the out of date background check. 9.22.22 2. Director emailed the background check form to board member with instructions that it needed to be submitted as soon as possible. 9.22.22 3. Board member will mail her completed background check form to DSS in Richmond no later than 9.27.22. 4. Director will email licensing inspector a scanned copy of board member?s notarized background check form and report the date it was mailed. 5. Director will email licensing inspector the results of the background check as soon as they are received. 6. Director will review and update facility procedures to ensure due dates for background checks are not missed.
