Please send documentation that the unmet Subsidy Health and Safety requirements pertaining to background checks have been corrected within 10 days of the inspection.
Corrections can be documented with scanned and emailed documentation,or may be faxed, or delivered in person to the Licensing Office. Mail delivery may be utilized ?at your own risk? if the correction is made in a timely manner allowing adequate time for mail to ARRIVE in the licensing office within the 10 day period.

A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on February 24, 2020, from 11:15 a.m. to 1:50 p.m. At the time of arrival the following staff to child ratios were observed:
7 Children, 30 months to 2 ? years old, with 1 staff
9 Children, 4 years old, with 1 staff
18 Children, 4 years old, with 3 staff
Several of the part time (3 and 4 day) programs were not in operation today
Children were observed resting quietly at nap time, engaging in small group learning centers, being read to and having an afternoon snack.
Eleven staff records were reviewed, posted information and parent handbooks were reviewed and the center program?s administrator was interviewed regarding the centers policies and procedures. The facility, to include all indoor areas used by preschool children and before and after care children, as well as the outdoor play space, were inspected.
The administrator reports that the program does not routinely transport preschool children in a vehicle.
