Based on a review of the playground, the facility failed to ensure that there is resilient surfacing complies with minimum safety standards. Evidence: 1. There is less than 1 inch of mulch at the base of the slide.
Plan of Correction:
The program director will ensure that the area is raked to comply with required 6 inches of resilient surface material before the children use the equipment.
