11/18/2019 Arrived 10:10am Departed 11:50am An unannounced monitoring inspection was conducted today. The children were playing outdoors, working in centers, reading stories and eating lunch. Previous violations, seven children's files and medications were reviewed. Fifty children were in care with six classroom staff, three center aides and the family health advocate. Staff files were not reviewed today. Please contact me if I may be of service. Thank you for your cooperation. 276 608 3749
Based on a tour of the playground, the facility failed to ensure that all equipment is maintained in a safe condition. Evidence: 1. The middle board on the lower wooden deck that goes to the yellow slide is worn and cracked, presenting splinters.
Plan of Correction:
The site administrator will ensure that the board is repaired or replaced.
