Comments:
An unannounced monitoring inspection was completed on 08/29/2019. There were 15 children, ages 3 years-4 years, and 2 staff present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 2 staff; the complete staff records will be reviewed at the corporate office on a later date. The provider stated that medication is not being administered at this time. Children were observed in the following activities: during free choice/center play, during story time, in large motor play outside, during lunch and during rest time. The inspector reviewed the emergency evacuation and shelter-in-place plans and the emergency supplies during the inspection. The inspector discussed children?s records and ointments with the staff. Description: Based on observations, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.


Evidence: The area underneath the climbing dome and at the chute of the slide did not contain the required 6" of resilient surfacing; dirt was visible in multiple areas underneath the climbing dome and was visible at the bottom of the slide chute. The children were observed playing on the playground, climbing on the dome and using the slide during the inspection.

Plan of Correction: To ask director to purchase more pea gravel and place it on the playground as soon as possible.

Description: Based on observations, the facility failed to ensure that ground supports shall be covered with materials that protect children from injury.

Evidence: The ground supports of the see-saw piece of equipment were exposed. The children were observed playing on the playground, including this piece of equipment, during the inspection.

Plan of Correction: To ask director to purchase more pea gravel and place it on the playground as soon as possible.
