Comments:
An unannounced monitoring inspection was conducted on 02/26/2020. There were 14 children, ages 3 years-5 years, and 2 staff members present during the inspection. The inspector reviewed 5 children's records and the on-site staff record requirement for 2 staff members during the inspection. The staff stated that there is currently no medication on-site. The children were observed in the following activities: in music and movement, during circle/learning time, participating in an art project and in free choice activities. The inspector reviewed the emergency supplies during the inspection. The inspector discussed rest time and snack time with the staff. Based on record review and discussion with staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence: 1. The record for Child #2 did not contain the following information: the work phone number of each parent who has custody (a place of employment was provided however the record did not contain the work phone number) and the health information as required by 22VAC40-185-140. 2. The record for Child #5 did not contain the work phone number of each parent who has custody (a place of employment was provided however the record did not contain the work phone number).
Plan of Correction:
The parents will be contacted for the missing information.
