Technical Assistance:
Technical Assistance offered in today's visit: background checks, children's immunization records and staff records.

Comments:
An unannounced monitoring inspection was conducted from 10:40 am-1:40pm. 40 children were in care of 7 staff members present. Children were observed playing with the sensory table, home living toys and computers, playing outside on the playground, eating lunch and taking naps. The lunch menu included meatballs, corn, pears and milk. The school age children were observed leaving for a field trip. 5 children's records, 5 staff records, injury reports and permission forms were reviewed. Violations:
Standard #: 22VAC40-185-90--A
Description: Based on record review, a written agreement between the parent and the center was not obtained to include authorization for emergency medical care, center's illness policy and parents notifying the center of any communicable disease at home within 24 hours. Evidence: Child # 1's record did not have a written agreement that included authorization for emergency medical care, center's illness policy and policy about parents notifying the center of any communicable disease at home within 24 hours.

Plan of Correction: I will get the authorizations from the parents.

Standard #: 22VAC40-185-210-A
Description: Based on record review and staff interview, a staff member did not meet the requirements to be a program leader. Evidence: 1. Staff # 1 stated that Staff # 2 was the lead teacher for the class with the three and four year old children. 2. Staff # 2's record only had high school transcripts. Staff # 2's record did not have any other documentation to prove that she was qualified to be a lead teacher.

Plan of Correction: A lead qualified teacher will be used to cover that classroom.

Standard #: 22VAC40-185-520-B
Description: Based on observation, sunscreen was not inaccessible to children under the age of five years. Evidence: A tube of sunscreen was found sitting on the diaper changing pad in the 2 year old classroom. The sunscreen was easily accessible to the children present.

Plan of Correction: Staff will be retrained.

Standard #: 22VAC40-185-520-D
Description: Based on observation, insect repellent was not inaccessible to children under the age of five years. Evidence: A bottle of insect repellent was found sitting on the diaper changing pad in the 2 year old classroom. The bottle was easily accessible to the children present.

Plan of Correction: Staff will be retrained.
