Comments:
A monitoring inspection was initiated on 3/12/2024 and concluded on 3/12/2024. There were nine children present with two caregivers supervising. The point total was 20. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 0? child records and three staff records were reviewed. The inspection started at 09:10 am and concluded at 11:50am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.058
Description: Based on observation, the provider did not ensure that a carbon monoxide detector was equipped in at least one room.
Evidence:
There was no carbon monoxide detector found in the home and the provider was unable to locate one.

Plan of Correction: Provider will purchase and install a carbon monoxide monitor.

Standard #: 8VAC20-800-900-A
Description: Based on observation, the provider failed to serve grapes cut in small pieces to prevent choking.
Evidence:
The provider served whole grapes to children under four years of age.

Plan of Correction: Provider stated they would cut grapes into smaller pieces the next time grapes are served.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to ensure that poisonous materials be inaccessible to children.
Evidence:
A bottle of pesticide, Ex-Lax and Bleach were found in unlocked cabinets in the kitchen which is adjacent to the room where children eat.

Plan of Correction: Provider moved items to a locked location during the inspection.

Standard #: 8VAC20-800-350-E
Description: (REPEAT VIOLATION) Based on observation, the provider failed to ensure that the hot water at taps available to children shall be maintained within a range of 105 degrees Fahrenheit to 120 degrees Fahrenheit.
Evidence:
The hot water measured 124.2 degrees Fahrenheit.

Plan of Correction: Provider stated the temperature would be lowered.

Standard #: 8VAC20-800-530-I
Description: Based on observation, the provider failed to use a mattress covered in a waterproof material that can be cleaned or sanitized.
Evidence:
A mattress was found in a crib used for infant nap time had two cracks in it revealing a non-absorbent material.

Plan of Correction: Provider stated they had a new one and would replace it immediately.
