Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An unannounced renewal inspection was initiated and concluded on May 18,2020. The provider was contacted by telephone to initiate the inspection. There was (1) child present (1 point) and (2) staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with an applicable standard and the violation was documented on a violation notice issued to the family day home.
If you should have any future questions, please call or send an e-mail to charles.perkins@dss.virginia.gov. Thank you.

Based on review, 2 of 2 staff files indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) documentation, in accordance with the requirements in 22 VAC 40-111-170, every two years since the most current documentation on file.
Evidence - 1. The Provider stated (during an interview conducted on the date of inspection - 05/18/2020) that the most recent TB documentation on file were dated - For Staff #1 TB documentation was dated - 01/29/2018; and for Staff #2 TB documentation was dated - 03/28/2018. 2. This same documentation was viewed during an UMI that was conducted on 01/06/2020.
