Comments:
The licensing inspector completed a scheduled modification study on Friday January 3, 2020 from approximately 10:30 am until 11:45 am. The Department received a request to modify the facility's license on December 13, 2019. The licensee has requested to relocate to Richmond County Elementary School located at 361 Walnut Street, Warsaw for after school care. The licensing inspector completed an inspection of the building located at this address and was assisted by the director of the program. Cafeteria C & G and the main gymnasium, along with the bathrooms and outdoor play areas will be used during operating hours. All areas of the facility were inspected. The space has the required furniture and age appropriate materials for the anticipated number of children in care. The inspector reviewed changes to the policies and emergency preparedness plan. No hazards were identified. At this time, the facility's license will be modified to allow 130 children to be cared for at this facility. The children will be 4 years, 10 months through 12 years of age. The effective dates of the license will not be affected. If you have any questions, please contact the inspector at (804) 662-9758.
Based on review of the first aid kit, the center did not obtain all required items for the first aid kit.
Evidence:
The first aid kit did not contain a second triangular bandage and first aid instructional manual as required.
Plan of Correction:
Per the director: I will purchase the second triangular bandage and first aid manual for the kit.
