This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A subsidy inspection was initiated on December 13, 2021 and concluded on December 15, 2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review the vendor failed to ensure that each staff has current first aid certification within the required time frame. Evidence: 1 - The records for staff 2 and 3 did not contain documentation of any first aid training prior to December 14, 2021. a. Staff 2 has a documented date of hire as July 24, 2021. b. Staff 3 has a documented date of hire as March 19, 2021. 2 - Staff 1 confirmed that staff 2 and 3 did not obtain the required first aid certification within the allotted time frame.
