Comments:
An unannounced renewal inspection was initiated and concluded on November 16, 2023. There were 30 children present ages two through school age with six staff. Children and staff records were reviewed. Children were observed in teacher and self-directed activities including nap, snack, outside play and table top activities. A walking of the facility inside and outside was completed. Required postings, first aid kit, emergency equipment, fire and health inspections, insurance and medications were reviewed. Violations regarding out of state background checks and first aid/CPR training were found, were discussed with the owners and appear on the violation notice.
Based on record review and interview the center failed to obtain out of state background checks as required. Evidence: 1 - The record for Staff #2 did not contain out of state background checks for states in which the staff has lived within the past five years. a. Staff #2 has documented date of hire as September 11, 2023. 2 - Staff #1 confirmed that the required out of state background checks had not been obtained as required.

Evidence: Staff #1 stated that three out of the four new staff records reviewed contained cardiopulmonary resuscitation training that was obtained by completing an online only course.
