Technical Assistance:
22 VAC40-111-90 Immunizations for Children. B Pursuant to subsection C of the Code of Virginia, documentation of immunizations is not required for an child whose: 1. Parent submits an affidavit to the family day home on the current form approved by the Virginia Department of Health stating that the administration of immunizing agents conflicts with the parent's or child's religious tenets or practices; or 2 Physician or local health department states on a Department of Health approved form that one or more of the required immunizations may be detrimental to the child's health, indicating the specific nature and probable duration of the medical condition or circumstance that contraindicates immunization. pg 9

Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, February 5, 2020 between 1:00 pm to 2:44 pm to determine this family day home compliance with licensing standards. During the inspection, there were observation of 10 children in care totaling 24 points with three caregiver present. The licensing inspector reviewed the designated children care areas, bathrooms, kitchen area, and emergency supplies at the family day home to determine compliance with licensing standards. During the inspection, the licensing inspectors completed observations of the children participating in various activities to include asleep in cribs, sitting at table with peers after lunch, interacting with caregivers, completing bathroom breaks, and transitioning into napping. There were four children's, one caregiver's, one assistant caregiver's, and two household member's records reviewed during this inspection. The licensing inspector reviewed the reviewed the required postings and emergency supplies. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of caregivers record and interview, the provider did not ensure to obtain a current tuberculosis screening for each staff.

Evidence: In Caregiver#2 record, the last dated TB screening was 02/08/17. Interview revealed that there was no current TB screening available for review during the inspection.

Plan of Correction: The provider will have caregiver complete by 2/11/2020.
