Comments:
his inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on February 19, 2021. The director was contacted by telephone to initiate the inspection. There were 101 children present and17 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff resubmit tuberculosis (TB) test results every two years. Evidence: Staff #1 TB test results was more than two years old . Staff #1TB test results dated 05/13/2018.
Plan of Correction:
I will notify a the staff person that they must get a new TB test by March 5TH . She will remain off the schedule until it is taken care of.

Description:
Based on review of injury records, Licensee did not ensure all required information was included in injur .
Evidence: Injury report for child #1 was incomplete. The injury record for Child #1 and Child #2 did not contain date and time parent was notified.
Plan of Correction:
We will reiterate with staff the proper way to refill out these reports. We will also have the assistant director and director will check them when they are filled out.
