An unannounced monitoring inspection was conducted to investigate a self-reported incident regarding supervision that occurred on March 15, 2022 during the operation of the unlicensed short-term program. The inspection was conducted on March 24, 2022 beginning at 11:00 AM and ending at 1:15 PM with the program director. Staff members were interviewed and documentation was reviewed. Based on the information gathered during the investigation, the self-reported incident regarding supervision was determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.
Based on staff interviews, the center failed to ensure that all children under 10 years of age were within actual sight and sound supervision of staff at all times. Evidence: On March 15, 2022 at approximately 2:30 PM, Child A, 5 years of age, was left alone at Patriot Park, a community park located near Ron Rosner YMCA Recreation Center. According to information obtained through staff interviews and documentation, the child was out of staff sight and sound supervision for approximately 7 minutes.
Plan of Correction:
The facility will be doing refresher training for all child care staff (including transitions and missing child) as well as enforcing headcounts to be written down throughout the day.
