Comments: Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on September 11, 2019 as a result of a self-reported incident that was received by the Valley Licensing Office. Interviews were conducted with two staff members and documentation reviewed. There were no violations found during this inspection.
