Comments:
An unannounced monitoring inspection was conducted on October 21, 2019 from approximately 3:40 pm to 5:50 pm. Two staff were present with twenty-six children. Five children's records were reviewed. Information to be maintained on site and training were reviewed for the two staff present.
Based on review, facility did not ensure there is a annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The most recent fire inspection report available for review was dated April 6, 2017.
Plan of Correction:
Site Director will contact area manager for area manager to get a copy of the inspection.

Based on observation and interview, facility did not ensure medication is kept in a locked place.
Evidence: A prescription medication was stored in a first aid kit. Site Director stated the medication was not in a locked place on this date as the lock on the cabinet was not working.

Evidence: Emergency evacuation and shelter-in-place procedures/maps were not posted in the facility. Site Director confirmed the documents were not on the wall where they used to hang.
