Comments:
An unannounced monitoring inspection was conducted on August 19, 2019 from approximately 9:30 am to 12:00 pm. Two staff were present with twenty-four children. Five children's records were reviewed. Information related to staff was reviewed for the two staff present.
Based on review, facility did not ensure 1 of 5 children's records has a physical examination within a month of beginning attendance.
Evidence: The record for child #3 (start date June 18, 2019) did not have a physical. Missing information was verified by staff member in charge.
Plan of Correction:
Staff will contact the school and see if they can get a copy. If they cannot, parents will be contacted.
