Comments:
An unannounced monitoring inspection was conducted September 12, 2019 from approximately 7:30 am to 9:00 am. Two staff were present with thirty children at the onset of inspection. Additional children and staff arrived during inspection. Five children's records were reviewed. Information maintained on site was reviewed for two staff present.
The providers responses for the ?Description of Action To Be Taken? were not received as of September 24, 2019 and will not appear on this ?Violation Notice?.
Based on review, facility did not ensure 1 of 5 children's records has documentation of required information.
Evidence: The record for child #5 (start date September 3, 2019) did not have an address for the second emergency contact.
