Comments:
An unannounced renewal inspection was conducted February 18, 2020, from approximately 10:40 am until 1:30 pm. Upon entrance there was 4 staff member with 29 children. There were additional staff on site for administrative purposes. The sample size of records reviewed included 5 children's records and 5 staff records. There are no medications being administered as a part of this program. The climbing wall is not being used as apart of the outdoor play area. Areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed and verified with Program Administrator during the exit interview. Violations from the previous inspection were reviewed. The inspection results will be reviewed with the Licensing Administrator for license issuance. Please contact the licensing inspector with any question (757) 404-0568.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of 5 staff records and provider interviews, the licnesee did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Evidence: The record for staff #4, date of hire 8-30-18 did not include a TB screening as confirmed by the Program Administrator.

Plan of Correction: The Program Administrator will ensure the staff obtain the TB screening within the week.

Standard #: 22VAC40-185-430-C
Description: Based on observation and staff interviews, the licensee did not ensure that play equipment used by children shall meet all the requirements.

Evidence: The S-hooks on the outdoor swings were reviewed and it was determined that 8 out of 16 of the S-hooks were open more than the thickness of a penny. The climbing wall /volcano was observed and the inside mesh area where the children can stand and or sit has 4 holes they measure between 1 1/2 inches to 4 inches in length and width.

Plan of Correction: The Program Administrator will ensure the climbing wall/volcano is off limits to the children and the swing hooks will be replaced.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and provider interviews, the licensee did not ensure the center shall maintain and include all required information for injury reports.

Evidence: Six injury reports were available for review,and determined incomplete as they did not include all required information. Six reports 6 did not include the date the parent was notified of the injury, 2 did not include a plan to prevent re-occurrence of injury, 1 did not include the 2nd signature, 1 did not include the time the parent was notified of the injury and 1 did not include how the parent was notified of the injury.

Plan of Correction: The Program Administrator will update the form today to include missing items. The staff will be retrained to communicate information to parents and fill out the form.
