Comments:
An unannounced renewal inspection was conducted beginning at 9:50 a.m. and ending at 1:00 p.m. with the director. There were 31 children in care ages 3 through 5 years old under the care and supervision of 6 staff. Children were observed participating in "Dr. Seuss Day" creating a picture of cat and the hat, reading Dr. Seuss stories and wearing their Dr. Seuss hats. Snack was observed. Food is brought from home.
Seven children's records, seven staff records and two agent records were reviewed. Emergency drills were up to date. Five medications were reviewed. There is a complete first aid kit, battery operated radio and battery operated flash light on site. Staff training hours were reviewed. The playground was observed.

Based on review of documentation, it was determined that the center did not have documentation of a sworn statement for all agents of the organization. Evidence: Agent #2 did not have documentation of a sworn statement on file.

Documentation was misfiled. Agent #2 completed the sworn statement during the inspection.

Description:
Based on review of staff records, it was determined that the center did not obtain central registry background check results within 30 days of employment. Evidence: Staff #2, hired 8/20/2019, central registry background check was dated 10/30/2019. Staff #5, hired 9/9/2019, central registry background check was dated 12/7/2019.
Plan of Correction:
The staff had central registry background checks through the Diocese and then we were informed of a procedure change and began conducting on site central registry checks.
