Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 12/15/20 and concluded on 12/18/20. The director was contacted by telephone to initiate the inspection. There were 35 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence: 1. The record for child 1 (date of enrollment: 10/20/20) does not contain documentation of a physical examination. 2. The youth development director confirmed the record for child 1 is lacking documentation of a physical exam.
Plan of Correction:
The center responded with the following: The parent of child 1 had turned in documentation of a physical that was not signed by the physician, which was rejected by the online registration system. The parent was contacted on 12/16/20 to remind them that documentation of the child's physical was still needed. Administration will contact the parent again to obtain a copy of the physical.
