Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A monitoring inspection was initiated on 10/7/2021 and concluded on 10/19/2021. The director was contacted by email to initiate the inspection. There were 105 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. There were no immunization records available for child #4 and child #5 at the time of the inspection. Both children were present during the inspection. 2. Staff confirmed that these records were not available

Staff will ensure all children's records contain immunization documentation.

Description:
Based on record review and interview it was determined the center did not obtain documentation that each child has received a physical examination by or under the direction of a physician within the required time frame. Evidence: 1. There were no records of physical exam available for child #1, child #4, and child #5 at the time of the inspection. These children were present during the inspection. 2. Staff confirmed that these records were not available

Description:
Based on record review and interview it was determined the center did not ensure that there was a record for each child enrolled at the center containing the required components. Evidence: 1. The records available for child #1, child #4 and child #5 were incomplete at the time of the inspection. These children were present during the inspection. 2. Staff confirmed that these records were missing required components.
