Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:45am. There were 12 children for a total of 33 points directly supervised by 3 staff. The home, 4 staff records, 4 children?s records, 1 household member?s record, attendance log, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside and outside and handwashing was observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, it was determined the Provider did not review and update the Emergency Preparedness Plan this past year. Evidence: 1. The Emergency Plan has been updated 5 times but not in the last year.

Description:
Based on interview and record review of the listed substitute/assistant staff member, it was determined documentation of the orientation was not signed and dated by the provider and this staff person. Evidence: 1. Provider stated that a household member is an assistant/substitute and children's records have this staff listed as the substitute provider. 2. The file for this staff member does not include documentation of Orientation.

Description:
Based on interview and record review it was determined that In addition to satisfactory completion of first aid training and CPR training, not all caregivers have obtained a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence: 1. Provider stated a household member is used as n assistant when necessary and Provider acknowledged this person is also listed as the substitute Provider. 2. The records for this staff member included CPR/First Aid certification but there was no other documents to verify 16 hours in annual training. 3. provider stated this staff person has not taken any other trainings except CPR and First Aid.
Plan of Correction:
I didn't realize it was necessary when this person is only used in emergencies. I will make sure training is completed.

Description:
Based on observation, it was determined that an area was not maintained clean. Evidence: 1. Observation of the children's bathroom, revealed a toilet had not been flushed after a child had used it.
Plan of Correction:
Corrected at the time of the visit and will continue to remind children and staff to flush after use.
