Comments:
An unannounced monitoring inspection was conducted today between 9:00 am a and 11:30 am. There were 69 children under direct supervision of 15 staff. The physical plant, children's records, staff records, evacuation drill logs, attendance logs, emergency plan, emergency supplies and medications were inspected. The children were observed hand washing, eating snack, attending a musical program and conducting a survey about pets. Morning snack consisted of raisin toast, oranges and water. Based on a review of children's records, the enterr failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child's first day of attendance.
Evidence: Child @1's record does not contain documentation of immunizations on the first day of attendance. The child's first day of attendance was 8/26/2019.
Plan of Correction:
I have already requested form from parent, parent will give to office 1/10/2020.
