Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential guidance for Child Care Programs.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on January 11th, 2021 The Director was contacted by telephone to initiate the inspection. There were 52 children present with 15 staff. The inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete .The information gathered during the inspection determined non-compliance with standards or law and violations were documented on the violation notice issued to the facility. Based on a review of student records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend.
Evidence: Child #1's record does not have documentation of immunizations on the first day of attendance. The child's first day was 09/16/2019, but immunization records are dated 09/24/2019.

Immunization records in child's file.
