Comments:
An unannounced monitoring inspection was conducted on August 14, 2023 from approximately 10:40 am to 12:10 pm. Four staff were observed supervising thirteen children, ranging in age from eight months - two years. Children were observed playing freely, participating in story time and preparing for lunch. Four child and one staff record were reviewed. Violations found are noted on the violation notice.
Based on review, facility did not ensure a staff member have a TB test or screening dated within 30 days prior to beginning employment.
Evidence: The record for staff #1 (start date 7/12/2023) had a TB screening dated 5/22/2023.
