Comments:
An unannounced monitoring inspection was conducted today. All rooms are in ratio during the inspection. Children were resting quietly in each room. 2 staff and 4 children's records were reviewed. Emergency evacuation drills, first aid supplies, injury reports, medications and emergency supplies are all in compliance. LI was present today from 1:05 PM - 3:20 PM.
Based on a review of records, one child's record does not include documentation of physical, past 30 days of enrollment. Evidence: Child 1's record does not contain documentation of a current physical. (Date of enrollment: 6/2/19)

I will ask the parents to provide the information as soon as possible.

Description:
Based on a review of records, children's records do not include all required information. Evidence: Missing information includes: *1 of 4 records reviewed did not include documentation of an acceptable form of Proof of Birth having been viewed. (Child #4) *2 of 4 records reviewed were missing documentation of father's information. (Child 2,3)

Description:
Based on a review of documentation, time spent on stomachs is not being documented for one infant, for which it is required. Evidence: Child #3 has no tummy time documented.
Plan of Correction:
Staff are doing the tummy time, just not documenting it. I will remind all staff to document tummy time.
