Comments:
An unannounced renewal inspection was conducted on 9/25/2019 between the hours of 7:00am and 8:30am. 29 children were in care with 4 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, medications, the Emergency Preparedness Plan, 5 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The children were observed playing video games, playing basketball, making necklaces, and building with legos. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice. A subsidy health and safety inspection (SHSI) was also conducted today. Description: Based on review, 1 of 5 children's records did not contain the required information.

Evidence:
1. The record for Child #2 did not contain documentation of the mother's work phone number.

Plan of Correction: Corrected during after school session.

Standard #: 22VAC40-185-70-A
Description: Based on review, 2 of 4 staff records did not contain required information.

Evidence:
The name, address and telephone number of a person to be notified in an emergency for Staff #1 and Staff #2 was not available for review.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Laundry detergent was observed in the bottom of an unlocked closet located in the room immediately upon entering through the designated entrance.
2. The room in which the closet was located is used by children.

Plan of Correction: Corrected during inspection. Staff shut and locked door after getting supplies.
