Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 10/16/2020 and concluded on 10/19/2020. The center supervisor was contacted by telephone to initiate the inspection. There were 15 children present and 13 staff. The inspector emailed the center supervisor a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, the center did not have documentation that 1 child had a physical examination by or under the direction of a physician before the child's first date of attendance.
Evidence: The record for Child #1 (start date 9/8/15) did not contain documentation of a physical exam.
Plan of Correction:
Documentation of the child's physical may be at another SACC location as the child did not regularly attend this school prior to the beginning of the SRS program this year. Center made contact to obtain a copy of the physical for the child's record.
