Comments:
An unannounced renewal inspection was conducted today from 10:45am-11:50am. There were 8 children directly supervised by 2 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed painting, playing outside, and preparing for lunch. Areas of non-compliance are identified in the Violation Notice. Based on record review, 1 of 3 children's records did not contain documentation within 30 days of attendance that the child had a physical examination by or under the direction of a physician.
Evidence: 1. Child #1 began attending the center on 8/21/2023. The record for Child #1 did not contain documentation of a physical exam.
