Technical Assistance:
Discussed updated medications and forms, and the Operations manual.

Comments:
An unannounced monitoring inspection was conducted today from 11:10am-1:00pm. There were 58 children (5-12yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for an afternoon party and eating lunch. Hand-washing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, 1 of 5 children's records did not contain the following information:

1. Child #2's (age: 7 years old) record did not contain documentation of a complete address for the second emergency contact person.

Plan of Correction: We will speak with the parents and have them give us the address and let them know it is a licensing requirement.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the program did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In classroom #152, a can of Lysol aerosol spray was sitting out on top of the children's cubby shelves.

Plan of Correction: Corrected during inspection.
