Comments:
An unannounced renewal inspection was conducted September 18 2019 between 7:15am and 8:30am. At the start of the inspection there were 18 children in the direct care of 5 staff members. Children were observed creating a Popsicle stick art project, playing table tennis, playing card games, and reading. The center is clean and organized. The center provides age-appropriate materials and activities for the children in care. Areas of non-compliance are listed on the violation notice. Based on record review, 1 of 6 children's records did not contain documentation of all of the required information.
Evidence: Child #5 record does not contain documentation of parent information (names, employment, telephone numbers, addresses) nor does it contain documentation of two emergency contacts.
Plan of Correction:
Will follow up with parents today to bring the information in.
