Staffing records are reviewed bi-annually at the main SACC office.
Comments:
An unannounced monitoring inspection was conducted on January 28 2020 between 9:00am and 10:15am. There were 47 children in the direct care of 3 staff members. Children were observed creating artwork, playing on electronics, building and reading. Areas of non-compliance are listed on the violation notice. Evidence: Child # 3 record does not contain parental contact information and emergency contact information.

Based on record review, 3 of 5 child records reviewed did not have documentation that the center requested parent confirmation that their child's record is up to date.
Evidence: Child #1 record last had documentation of a review August 28 2014, Child #2 record last had documentation of a review September 1 2014, and Child #5 record last had documentation of a review September 6 2016.
Plan of Correction:
Will go through each file and follow up with each parent and have them sign off that the record is up to date.
