Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 05/11/2020 and concluded on the same day. The director was contacted by telephone to initiate the inspection. There were 2 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, one of two files did not contain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1) The record for Child #2 (dob 01/18/2016) did not contain a current immunization record. The most recent immunization report available for review was dated 11/15/16.
Plan of Correction:
I contacted the mother and she will provide the updated record from the doctor and get it to us ASAP.
