Technical Assistance:
Putting child proof latches or locks on the master bedroom and bathroom doors was discussed as family medication and hygiene products are kept in these rooms.

Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 1:00 PM and departed at 2:05 PM. Four children were in care with the provider. Nap time was observed. Two provider/household records and four children's records were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported.

An exit meeting was conducted with the provider prior to closure of the inspection.

Violations:
Standard #: 22VAC40-180-120-C
Description: Based upon review of records and provider interview, the provider has not ensured that documentation of a physical examination is obtained within thirty days of attending and that monthly evacuation drills are conducted and documented.
Evidence:
1. The record provided for child 3 who, according to the provider has been attending for well over thirty days, did not include documentation of a physical examination conducted by a physician.
2. the provider verified that a physical examination was not in the record for child 3.
3. The provider stated that evacuation drills have not been conducted since the summer.

Plan of Correction: The provider responded with the following:
The child's parent was notified that documentation of a physical examination must be provided.
Monthly evacuation drills will be conducted and documented.
