Comments:
An unannounced monitoring inspection was conducted on 10/10/2019. The inspection began at 9:30 AM and concluded at 12:30 PM. There were eleven children in care with two caregivers with a total of thirty points. During the inspection fine motor activities were observed, shapes and manipulatives were observed, hand-washing practices, and lunchtime. Additionally, the inspector reviewed five children's records, the licensee?s record, the substitute record, and three assistant?s records. The inspector examined the home interior and exterior, emergency supplies, and the outside play area. This provider administers medications and transports children from school to the family day home; therefore the LI reviewed one medication and the provider?s vehicle. Based on record review, the provider failed to ensure that an assistant (4) had a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.
Evidence:
1. CPR documentation for assistant (4), was dated 08/10/2017 with a two year expiration. The provider verified that the individual (4) did not obtain training for CPR since 08/10/2017.
Plan of Correction:
The provider plans to send the employee for CPR training.

Description:
Based on record review, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff (3) had a documented report of tuberculosis screening/test of 06/10/2017. When the LI asked the provider (1) if the record (3) had a newer report, provider (1), said that they did not.
Plan of Correction:
The provider will obtain a copy of the test and/or get a TB screening or test.
