Comments:
An unannounced inspection was conducted on 7/3/2024 in response to a complaint that was received by the licensing department on 6/21/2024 regarding allegations in the areas of supervision and injury reporting. On 7/3/2024, the inspector was on site from approximately 10:10 am to 11:10 am. There were 24 children in care, ranging in age from 12 months to 12 years, with 6 staff supervising. Interviews were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there was a violation found during the inspection regarding children's records. See the violation notice for more details.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Based on record review and interview, the center did not ensure that one child's record contained the required information.
Evidence: 1. The record of child #1 (date of attendance: 6/3/2024) did not contain documentation of viewing proof of the child's identity and age as required. 2. The provider acknowledged that the information had not been received.
Plan of Correction:
The director will not enroll children without proof of age and identity. I the director will call the local law enforcement agency (when required proof of identity is not provided.) This completion will take place immediately 7/5/2024.
