Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 11:50 am - 3:20 pm in conjunction with a monitoring inspection (see that report for details). SHSI requirements were reviewed. The last shelter-in-place drill was conducted: 7/15/19. Five medications were reviewed. Allergy action plans for three children with diagnosed food allergies were reviewed. Based on a review of documentation, not all children with a diagnosed food allergy had a written allergy action plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #1 had an allergy action plan that did not have a physician's signature. Child #2, with a diagnosed food allergy, did not have a written allergy action plan.
Plan of Correction:
Will ask the parents to provide an allergy action plan.

Description:
Based on a review of five staff files, not all staff had a statement that the individual is determined to be free of communicable tuberculosis (TB) submitted at the time of employment and prior to coming into contact with children. Evidence: Staff #3 did not have documentation of a negative TB test/screening in their file.
Plan of Correction:
Employee had the TB test/screening last Friday. Will obtain the results and place in their file.
