All staff must complete the three hour Virginia Pre-service Update training.
Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 9:00 am - 2:00 pm with the center director. A monitoring inspection was also conducted (see that report for details.) The last lock down drill was conducted: 7/9/19. There were three children with diagnosed food allergies and three allergy action plans and eight medications were reviewed. Based on review of eight children's records, not all children with diagnosed food allergies had documentation of a written allergy care plan with instructions from a physician. Evidence: the records for child #10 and child #14 did not have a written allergy care plan signed by a parent and physician.

Description:
Based on review of eight staff records, not all staff had documentation of a negative Tuberculosis (TB) test/screening at the time of employment and prior to coming into contact with children, that was completed within the last 30 calendar days of the date of employment and signed by a physician, physician's designee, or an official of the local health department. Evidence: the TB test/screening for staff #3 (date of hire: 3/9/20 was dated: 7/9/18 and had no signature; for staff #4 was not signed; for staff #6 (date of hire: 2/19/19) was dated: 3/4/19; and for staff #8 (date of hire: 3/3/20 was dated: 7/25/18.
