Comments:
An investigation was initiated on 7/10/19 in response to a self-reported incident received by the Eastern Regional Office on 7/9/19 regarding a child injury. An on-site inspection was conducted on 7/18/19. Interviews with staff were conducted and documents and records related to the incident were reviewed. Collateral statements were also obtained. The investigation was concluded on 9/23/19. Based on the information gathered during the investigation, the self-reported incident is determined to be not valid. One violation not related to the incident is identified on the violation notice and was discussed with administration during an exit meeting on 10/2/19.

Description: Based on observation and interview, the center did not ensure that hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The cabinet doors in the Green 1 classroom were missing, resulting in the cleaning materials being stored in them to be unlocked. A spray bottle of Lemon Brite all purpose cleaner with bleach, a spray bottle of bleach water and 2 cans of Vectair Airoma were found on the shelves of the cabinets and were accessible to children in care.
2. Staff confirmed that the cleaning materials were not kept in a locked area of the classroom.

Plan of Correction: The center responded with the following: Maintenance team has replaced all cabinet doors. All chemicals are locked and secured properly.
