Comments:
Licensing Inspector arrived at 10:00 and departed at 11:00 AM to conduct a voluntary registration renewal. At the time of the inspection, there was one child in care with the vendor. Child was observed watching television during the inspection.
Based on review of four children's records, it was determined that the provider did not ensure that all elements of the Health and Safety Checklist were in compliance.
Evidence: The record for child #2 did not contain immunizations or physical.
Plan of Correction:
The provider will obtain the immunizations and physical from the parent and place it in the file.
