A subsidy health and safety inspection (SHSI) was conducted on 07/20/2022 from 10:45am to approximately 11:30am. Two children were present and one staff. A total of four records were reviewed which were two children?s records, one staff member?s record, and a household member. First Aid kits and emergency preparedness documents were observed. The transportation van was observed. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. . The violations are listed on the violation notice issued and were reviewed with the vendor at the exit interview.
Based on record review and interview, it was determined that the vendor did not ensure that the child?s record had at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached including the name, address, and phone number. Evidence: 1. Child #1 did not have an emergency contact listed in their record. 2. Child #2 did not have a phone number listed for their emergency contact. 3. Staff #1 confirmed the emergency contact was not listed in the children?s records.
Plan of Correction:
The vendor will obtain the emergency contacts at pickup today.
