An unannounced complaint inspection was conducted on Thursday, October 17, 2019 in response to a complaint that was received by the licensing office on 10/10/2019. The inspector was on site from approximately 7:35 AM to 8:40 AM. Documentation was reviewed relating to allegations concerning staff to child ratio requirements.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on observation, the facility did not ensure compliance with staff to children ratio requirements. Evidence: 1. Based on observation the center did not maintain a ratio of one staff to every 8 children for children two years old. 2. Staff #1, in classroom #3, had 10 children two years of age at 8:05 AM on October 17, 2019. 3. At approximately, 8:08 AM staff #2 arrived in classroom #3.
